Set up an EPR for another person
To access the electronic patient record (EPR) of the person you are representing, you must identify yourself securely, for which you need an electronic identity (eID). An eID is a type of digital ID. You can choose the SwissID or the TrustID for your eID. Choose one of these eIDs. Then open an account and install the app on your smartphone.
Setting up an EPR for someone else currently requires a personal appointment.
Please bring all the documents mentioned below with you to the appointment:
- Proxy certification (family register, decision on guardianship, support, etc.)
- Filled in declaration of consent
- Copy of the ID of the person to be represented
- Own ID
- OASI number of the person to be represented (e.g. on health insurance card)
- Login for your eID account (SwissID or TrustID) and installed eID app
- Active smartphone with a valid mobile number
- Access to your personal email account
Make an appointment at the EPR opening centre "Central Services of Sanela". You will find the link for registration below.
The opening centre enters the details of the person to be represented and then sets up the EPR of that person. After successfully setting up the record, you can access and use the EPR at any time via a secure internet connection.
